The annual subscription for the PENTAGRAM journal is $60 (6 issues). A free complimentary copy is available on request.

Your subscription to the bimonthly journal PENTAGRAM will be arranged for you on receipt of your application and payment. 
Please complete and submit the form below and we will invoice you for a 12 month subscription, 
or you may print and post this form together with your payment to the centre nearest you.

The information you provide us is held in strict confidence. 

If you do not want us to retain your contact details for purposes other than supplying you the PENTAGRAM journal,
